Submission of the application for a visa issue:
Children under the age of 12 should not come to the visa center of Germany personally.
Children who are already 12 years of age should personally come to the German visa center in order to submit their biometric data (fingerprints).
Children between the age of 12 and 16 should not come for the documents’ submission personally if they have submitted the biometric data at least once during the last 5 years.
Please note: You can apply to VisaMetric for a visa issue without prior registration. However, in order to reduce the waiting time in the queue, please make an appointment beforehand. For each child, you will need to create a separate one.
For each child, you need to will have to fill-in a separate visa application form, which must be signed by persons entitled of the child’s custody (usually parents). Children who are already 16 years old additionally sign their visa application form by their own hand.
Minors (under the age of 18) should provide the following documents:
- International passport.
Children under the age of 16 can inscribed into the passport of one of the parents accompanying them during the trip. If the child is inscribed in one of the parents’ passports, it is necessary to provide a copy of the first page of the passport of one of the parents and a copy of the page with the personal data of the child.
Children not accompanied by their parents, or if they are already 16 years of age, need to have a separate travel document. - Child birth certificate (original and a copy).
- The original of the identity document (a citizen's passport or residence permit in Ukraine).
- If one of the parents or the minor changed their surname after receiving the birth certificate, they must provide documents confirming it, for example: a marriage certificate / divorce certificate or a Civil registry department certification (original of the document and a copy).
- The completed application form, signed by parents (paragraph 37 and the last two pages of the application form, usually pages 6 and 7). Children who are already 16 years old additionally sign their visa application form on their own hand.
If a minor travels with his parents, the following documents should be provided:
- confirmation of staying on a trip with a minor;
- round trip tickets for parents and the child;
In the case of valid visa / biomedical passports of parents:
- Copies of valid visas;
- the first pages of passports of parents with personal data and the originals of these passports.
If a minor travels with one parent, the following documents should be provided:
- confirmation of staying on a trip with a minor;
- round trip tickets for parents and the child;
- notarial exit permit, signed by another parent (original and copy), issued during the last 6 months, as well as a copy of his/her civil passport. The original of the notarial exit permit should be submitted to the Embassy/ General Consulate of Germany for review which shall be returned only after the passport issue. This permit must contain information regarding the fact that the person entitled to custody is responsible the child’s abroad trip. As a rule, the permit also specifies the purpose of the trip and how long this permit remains in force (for example, "vacation", "for a visit"). Also, such permit must indicate who will accompany the child during the abroad trip (for example, "accompanied by grandmother X", "unaccompanied, by Y company airplane; the child will be met at Z airport", "accompanied by my wife A").
If a person having the right of custody lives outside Ukraine, the exit permit for the child's abroad trip should be issued:
- at the Ukrainian foreign representative body or
- at the German administration or
- at the German foreign representative body or
- by a foreign notary with an apostille or legalization
If one of the parents who travels with the child has a valid visa/biometric passport:
- a copy of a valid parent's visa;
- a copy of the first page of the passport of one of the parents with his/her personal data.
If a minor travels with an accompanying person, the following documents should be provided:
- confirmation of staying on a trip with a minor;
- round trip tickets for an accompanying person and the child;
- notarial exit permit, signed by both parents (original and copy), issued during the last 6 months, as well as copies of their civil passports. The original of the notarial exit permit should be submitted to the Embassy/ General Consulate of Germany for review which shall be returned only after the passport issue. This permit must contain information regarding the fact that the person entitled to custody agrees with the child’s abroad trip. As a rule, the permit also specifies the purpose of the trip and how long this permit remains in force (for example, "vacation", "for a visit"). Also, such permit must indicate who will accompany the child during the abroad trip (for example, "accompanied by grandmother X", "unaccompanied, by Y company airplane; the child will be met at Z airport", "accompanied by my wife A").
If a person having the right of custody lives outside Ukraine, the exit permit for the child's abroad trip should be issued:
- at the Ukrainian foreign representative body or
- at the German administration or
- at the German foreign representative body or
- by a foreign notary with an apostille or legalization
If an accompanying person has a valid visa/biometric passport:
- a copy of a valid accompanying person’s visa;
- a copy of the first page of the passport of an accompanying person with his/her personal data.
If a child travels with his/her legal guardian, the following documents should be provided:
- confirmation of staying on a trip with a minor;
- round trip tickets for a legal guardian and a child;
- documents confirming the termination of parental rights or loss of parents;
- court judgment on the appointment of a guardian;
- consent from the child protection services.
If a legal guardian has a valid visa/biometric passport:
- a copy of a valid guardian’s visa;
- a copy of the first page of the passport of the guardian with his/her personal data.
If the parent or a child is absent/missing, the following documents should be provided:
- the original and a copy of the death certificate of the father or mother;
- certificate of deprivation of parental rights;
- if at the child’s birth the father’s data was included into the birth certificate on the basis of the mother's words, it is necessary to provide a certificate from the district Civilian Registry Office - form 25;
- a police certificate in case one of the parents disappears or his/her location is not established.
