You can choose to fill in one of the two application forms: printed version or online version.
Consider the following - the submission of the hand-written application form shall increase the documents’ submission time at the VisaMetric visa center (especially if the applicant needs to submit fingerprints), because in such case all data is being manually entered into the system by the manager during the documents’ reception.
Along with the application form it is required to submit the two following documents (they should be filled in, preferably in English or German language, and signed by hand, as well brought to the visa center along with all other documents according to your submission check-list):
Please note, that in case the documents are submitted by a third person (upon the condition that the fingerprints have already been submitted during the previous Schengen visa issue, or in case applicants are under 12) the letter of attorney, signed by the applicant, should be present:

Please, note, that the "Launch assistant" function can be enabled during the application filling.
After printing all pages of the application form, please, recheck the filled information and sign the application in three places. This online application form should be printed out along with the bar-coded page and submitted with other documents on the day of submission.
